Promotion begins START DAY, DATE Pacific Standard Time (PST) and ends at 11:59 P.M. PST on END DAY, DATE. Gelb Consulting will accept online entries via the Official Entry Form received by 11:59 P.M. PST on END DAY, DATE. Gelb Consulting will also accept entries by mail. No advantage will be given to one form of entry over another. Mail-in entries must be postmarked on or between START DAY, DATE , and END DAY, DATE, and received by 11:59 P.M. PST on END DAY, DATE. Limit: one (1) entry per person or email address. In the event of a dispute over the identity of an online entrant, Entry will be deemed submitted by the holder of the email account. In case of multiple entries from one person or email address, only the first valid entry received will be eligible. Photocopied entries, mechanical reproductions, or use of automated devices are not valid for entry. All entries become the property of Gelb Consulting and will not be returned or acknowledged. The Sponsor is not responsible for lost, interrupted, or unavailable network server or other connection, miscommunications, failed phone or computer or telephone transmissions or technical failure, jumbled, scrambled, or misdirected transmissions, or late, lost, damaged, or postage-due mail or other error of any kind whether human, mechanical, or electronic. Incorrect, incomplete, mutilated, multiple, late, and any other type of defective entry whatsoever will be automatically disqualified. CAUTION: Any attempt by an entrant to deliberately damage any Web site or undermine the legitimate operations of the Promotion is a violation of criminal and civil laws. Should such an attempt be made, the Sponsors reserve the right to seek damages from any such Entrant to the fullest extent permitted by law and to disqualify such Entrant from the Promotion. In the event the Promotion becomes technically corrupted in any way or is compromised by virus, bugs, non-authorized human intervention, or any other causes beyond the control of the Sponsor (which corrupt or impair the administration, security, fairness, or proper play of the Promotion), Sponsor reserves the right, at its sole discretion, to disqualify subsequent electronic entries, but to continue the Promotion with all mail-in entries and previously received valid electronic entries. ELIGIBILITY. All residents of the fifty (50) United States of America and the District of Columbia are eligible to enter and win, except employees and immediate family members of the Sponsors, their parent companies, divisions, subsidiaries, affiliates, advertising and production agencies, and any other persons involved with this Promotion. Entrants must be at least eighteen (18) years of age. Void where prohibited, taxed, or restricted. Governed by and subject to all U.S. federal, state, and local laws and regulations. Winner(s) may not substitute or transfer a prize. DETAIL PRIZE(S) HERE. The Sponsors reserve the right to substitute a prize for one of equal or greater value in the event that Prize is unavailable. One (1) prize per household. Prize is not redeemable for cash. ODDS OF WINNING. There will be NUMBER (#) winner. Odds of winning depend on the number of eligible online and mail-in entries received. WINNERS. Winners will be determined in a random drawing on END DAY, DATE , or shortly thereafter, from all valid entries received. Winners will be selected using a computer program. Winners will be notified by email, or if provided, standard mail at the street address provided by Entrant. If a Prize Notification email or letter is returned as undeliverable by either email or standard mail, it will result in disqualification, Sponsors will conduct a new random drawing thereby selecting an Alternate Winner, and the corresponding prize will be awarded to the Alternate Winner. This process shall continue until either all prizes have been awarded or no valid entries remain. At the sole discretion of the Sponsors, winners may be required to sign and return an Affidavit of Eligibility and Release of Liability/Publicity Release within fourteen (14) days of date printed on the Prize Notification Letter or he/she will be disqualified and an Alternate Winner will be selected as described above. Winners may be required to provide their social security numbers and/or tax identification numbers. Winners must meet all criteria in these Rules or face forfeiture of Prizes based upon the sole discretion of the Sponsors. Except where prohibited by law, Entry constitutes express permission to use winners' names, likenesses, hometowns, and prizes won for promotional purposes without further compensation. However, acceptance of a prize is not conditioned on Gelb Consulting's ability to use winners' names. DELIVERY OF PRIZES. Prizes will be sent by e-mail or regular mail within approximately four-six (4-6) weeks after the date of the drawing. One email notification will be made for which Sponsors will bear the cost. If a Winner has not received Prize within six (6) weeks of the date on the Prize Notification Letter, Winner may contact Fulfillment Manager, Gelb Consulting, to determine status of delivery. Winners who contact Gelb Consulting by telephone and reside outside of the local calling area may incur a long distance charge for their call. If a prize is undeliverable for any reason whatsoever, the Winner must contact Sponsors to arrange for redelivery at Winner's expense within two (2) weeks of the last delivery attempt, or forfeit the prize. If, after two weeks from the date on the Prize Notification E-mail, a Winner cannot be located, contacted, or fails to arrange for redelivery of a prize, that Winner forfeits the prize. LIABILITY/WARRANTY/RESERVATION OF RIGHTS. By entering this Promotion, all entrants agree that all prizes are awarded upon the express condition that the Sponsors and their respective agencies or employees have no liability whatsoever for any injuries, losses, or damages of any kind resulting from participation in this Promotion, or acceptance, possession, or use of any prize.
